Patient Information

To register as a patient please call at Reception and fill in a registration form.

Home Visits

If you are too ill to visit the surgery a doctor will visit you at home. It helps if the request is made before 10.30am. Please do not be offended if the receptionist asks you for details. On average seeing a patient at home takes four times longer than seeing a patient at the Health Centre, which is why, when possible, we prefer to see patients at the Health Centre

Disabled Patients

All our rooms are on ground level. We have automatic entrance doors and a disabled toilet

Chaperones

Please ask the receptionist if you require a chaperone for "intimate examinations"

If you care for a relative, friend or neighbour please let a member of staff know so that we can update your records.

The Practice treats all information given with full confidentiality, but at times information does need to be shared with other Health Care Services for the benefit of the patient. Full details of our Data Handling Policy is in leaflet form available from Reception.

Please be aware that the PCT may occasionnally contact patients to check the address that they hold for a patient is the same as on our own database.
If you do receive this letter, please rspond to the PCT or you will risk being deducted from our list by the Trust.

Your responsibilties as a patient

If you need to cancel a pre-booked appointment, please telephone reception to do this. Your appointment can then be used for another appointment. Our staff work hard to provide you with the best possible care.

Warning - Abuse of Staff

We will NOT tolerate verbal or physical abuse to our staff or damage to our property. All such incidents will be treated seriously and may lead to the removal of a patient from our list.
