Choose and Book

Choose and Book is a national electronic referral service which enables patients a choice of hospital, date and time for their first outpatient appointment. Patients can then book their own appointment to see a specialist. This can be done in the surgery if needed or at home by telephone or over the internet.
At your consultation with your doctor your choice of hospital will be discussed and the doctor will let the secretary know which hospital you would prefer to go to. The secretary will then create your Choose and Book referral and where possible a minimum of 3 hospitals will be offered including the one you have already chosen.
This is done to give you more choice in case your chosen hospital has a longer waiting time for an appointment or in case there are no appointments available on the day you try and book.
Your referral letter will be typed and signed by your doctor. This will then be attached to a URBN (Unique Reference Booking Number) which is created at the time the referral letter is typed and sent electronically to the Choose and Book's central booking office until you have made your appointment and then it will be sent electronically again to your chosen hospital.
The URBN (Unique Reference Booking Number) which is 12 digits long, choice of hospitals and your password will be printed and sent to you with full instructions and all the telephone numbers and internet address you need to be able book your appointment successfully. Please have this paperwork to hand when you book your appointment as you will be asked for your
URBN number and password.
